Haiqubopa Ethanolamine Tablets, also known as Hengqu, is a Class 1 innovative drug independently developed by Jiangsu Hengrui Pharmaceutical Co., Ltd. Since its approval by the National Medical Products Administration in June 2021, it has become an important drug for the treatment of adult patients with chronic primary immune thrombocytopenia (ITP) who have previously had poor response to treatments such as glucocorticoids and immunoglobulins due to increased risk of bleeding caused by thrombocytopenia and clinical conditions, as well as adult patients with severe aplastic anemia (SAA) who have had poor efficacy in immunosuppressive therapy. In December 2021, Haiqupopa ethanolamine tablets were officially included in the National Basic Medical Insurance, Work Injury Insurance, and Maternity Insurance Drug Catalogue (2021). However, as a prescription drug, patients need to strictly follow a series of precautions when taking Haiqupopa ethanolamine tablets to ensure medication safety and effectiveness.
1、 Basic information of the drug
The main ingredient of Haiqupopa ethanolamine tablets is Haiqupopa ethanolamine, which is an orally absorbable small molecule human thrombopoietin (TPO) receptor agonist. It can effectively stimulate the thrombopoietin receptor, increase platelet count, and reduce or prevent bleeding. The dosage form of this drug is a tablet, with various specifications including 2.5mg, 3.75mg, and 5mg. The specific specification for use depends on the doctor's guidance.
2、 Precautions for use
1. Dosage and adjustment
For adult patients with primary immune thrombocytopenia (ITP), the recommended initial dose is 2.5mg, once daily, taken orally on an empty stomach. During the treatment process, platelet count should be monitored and the dosage adjusted according to the platelet count. The maximum dose should not exceed 7.5mg per day. Dose adjustment should follow individualized principles to ensure that platelet counts are maintained within a certain range and avoid excessive elevation.
For patients with severe aplastic anemia (SAA), the recommended initial dose is 7.5mg once daily, taken orally on an empty stomach. Regular monitoring of platelet count is also necessary, and the dosage should be adjusted every 2 weeks based on the platelet count until the minimum dose required to maintain platelet response is reached. The maximum dose should not exceed 15mg per day.
2. Medication time and food effects
Hyraclopramide ethanolamine tablets should be taken orally on an empty stomach and should not be taken with meals. Eat at least 2 hours after oral administration to ensure full absorption of the medication. In addition, dairy products such as milk, yogurt, cheese, as well as supplements containing minerals such as aluminum, calcium, magnesium, and iron, also need to be used at least 2 hours after taking the medication to avoid drug interactions and affect the efficacy.
3. Monitor platelet count
After the first administration and any dose adjustment, platelet count should be monitored at least once a week for 2-3 weeks to observe changes in the patient's platelet count and consider whether to further adjust the dose. If the patient reaches dose stability (dose remains unchanged for 3 weeks), the frequency of platelet monitoring can be appropriately reduced (such as once every 2-4 weeks).
For ITP patients, if the platelet count does not increase to a level sufficient to avoid severe clinical bleeding after 4 weeks of treatment with a dose of 7.5mg once daily, the treatment with this product should be discontinued. After discontinuation of medication, blood routine including platelet count should continue to be monitored once a week for at least 4 weeks.
For SAA patients, if no hematological response occurs after 24 weeks of treatment, it is recommended to discontinue treatment with this product.
4. Adverse reactions and coping measures
Some patients may experience adverse reactions such as dizziness, vertigo, drowsiness, drowsiness, fatigue, etc. after taking medication. They should be vigilant and avoid driving or performing mechanical operations in a state of dizziness and lack of alertness.
Haiqupopa ethanolamine tablets may worsen cataracts or increase the risk of developing new cataracts. It is important to monitor eye reactions and report to a doctor in a timely manner.
If there is persistent diarrhea with complications (such as rectal bleeding or positive fecal occult blood), seek medical attention immediately or stop taking this product under the guidance of a doctor, and start symptomatic hemostasis treatment as soon as possible.
If any serious, persistent or progressive symptoms occur during medication, such as allergic reactions (rash, urticaria, itching, redness, swelling, blisters or skin peeling, fever, wheezing, difficulty breathing, etc.), liver problems (dark urine color, feeling tired, satiety, upper right abdominal pain or tenderness, light stool color, vomiting, yellowing of skin or eyes, etc.), thromboembolism (chest pain or tightness, shortness of breath, cough, swelling of legs or arms, fever, numbness, skin discoloration or pain, difficulty speaking or swallowing, etc.), medical attention should be sought in a timely manner.
5. Medication for special populations
Currently, there are no pharmacokinetic studies or clinical trial data available for patients with liver and kidney dysfunction. It is recommended that patients with liver and kidney dysfunction use this product with caution. Before starting treatment with this product, serum levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST), and bilirubin should be measured. If the patient has significant liver function test abnormalities before starting medication, it is not recommended to use this product for treatment.
Pregnant women should avoid using this product as it can pass through the placental barrier and may cause damage to the fetus.
Elderly patients do not require dosage adjustments, but close monitoring is still necessary during medication.
There is no clinical trial data available for this product in patients under 18 years old, therefore it is not recommended for children to use.
3、 Lifestyle adjustment
During the period of taking Haiqupopa ethanolamine tablets, patients should also pay attention to adjusting their lifestyle to assist in the treatment effect. It is recommended to maintain a healthy diet, have a regular schedule, avoid staying up late and smoking and drinking alcohol. Moderate exercise, maintain a happy mood, avoid overexertion and emotional fluctuations.
4、 Summary: Haiqupopa ethanolamine tablets, as an effective platelet boosting drug, provide a new treatment option for patients with chronic primary immune thrombocytopenia and severe aplastic anemia. However, patients need to strictly follow the doctor's guidance when taking the medication, pay attention to medication dosage, medication time, food effects, platelet count monitoring, adverse reaction response, and special population medication precautions. Meanwhile, adjusting lifestyle is also an important factor in ensuring the effectiveness of treatment. Only in this way can we ensure the safety and effectiveness of medication, help patients better control their condition, and improve their quality of life.
